Click on the numbers to complete the different sections of Annexure A2 failures.

Once completed click on Submit and on the next page click on print.

Select Adobe PDF or .XPS file format and save the document to a desired location on the C Drive.

Please note this template can only be used to create a pdf or microsoft .xps type document which must be uploaded to the BIO on your profile.
