Click on the icon below to download the Excel template for Annexure A1
The open or save ribbon will appear at the bottom of the screen.
If you click on Open and the Excel file does not open use the following procedure:
Click on Save as and save the excel file to a location on your c drive. Once you have downloaded the file, right click on the Excel file and select properties. Click on Unblock at the bottom and OK.
Please note: Once completed the Excel file must be saved as a PDF. The instructions are inside the Excel file.
Click on the button below if you do not have Excel installed . Fill out the form, right click and select Print Preview. Click on the third button on the top right to change to landscape. Click on Print and select Adobe PDF or .XPS format. Save the document to a desired location on your c drive.
